Registration
	All health and emergency/release forms are due on May 26. Registrations are accepted on a first-come, first-served basis. Some camps fill as early as March 1.  When camps become full, this will be indicated on the website and in the online registration feature. A limited number of Purchase of Care slots are available; contact cass_winner@wmsde.org to check availability. Specialty Camps may be cancelled in early May if registration is not sufficient.     To register, you may choose one of two options:   Register online 24/7 Fill out our online registration form. You will still need to download and fax or mail the Emergency Treatment/Release forms and the health appraisal. Register by mail/fax/email Download all registration forms below for manual paper or faxed/e-mailed registration. Call 302-475-0555 or email Cass_Winner@wmsde.org if you prefer to have forms mailed to you. Downloadable forms: Emergency Treatment/Release Form Health Appraisal Children will not be accepted into camp without the health form, which requires the signature or stamp of your child’s doctor.  If his/her annual physical is not until the summer, we will need a note to this effect, and to have our form filled out with the information from last year’s visit. If you have a copy of a similar form (signed by the doctor’s office) from school or daycare, you may fax or mail it to our school/camp nurse to see whether it would be an acceptable substitute.  Wilmington Montessori students with a current health appraisal form on file do not need to do another.  Returning families:  If your child attended Camp Montessori during summer 2011, you may return just the Selection Form and registration form for each child — make sure your email address is included, please — and we will send you copies of last year’s emergency information. You may just notify us of any changes.
