____Camper Registration Form/Swim/Sunscreen Form (1 per child)
____Camp Selection Form (1 per child)
____Emergency Treatment/Release Information Form (1 per child)
____Payment (as described on Selection Form)
____The Health Appraisal is due by May 26.
Children will not be accepted into camp without the health form, which requires the signature or stamp of your child’s doctor.
If his/her annual physical is not until the summer, we will need a note to this effect, and to have our form filled out with the information from last year’s visit. If you have a copy of a similar form (signed by the doctor’s office) from school or daycare, you may fax or mail it to our school/camp nurse to see whether it would be an acceptable substitute.
Wilmington Montessori students with a current health appraisal form on file do not need to do another.
Returning Families: If your camper attended Summer ’09, you may return just the Selection Form and registration form for each child, and we will send you copies of last year’s emergency information. You may just notify us of changes.
